Power of Attorney (Durable)

Description

A Power of Attorney form allows you to appoint another person to act on your behalf should you ever require someone to make short- or long-term decisions for you. "Durable" here implies a contract which remains in force even after the principal later becomes mentally incapacitated and ends automatically when the principal dies or is revoked by notifying your Agent in writing.
