How do you claim a dependent's benefits?

 If the husband or wife of the deceased is making the application they must register at the nearest Labour Office. 

 Documents you need to make the claim:

·   Your ID book

·   Copies of the deceased’s last six pay slips

·   UI-19 – information supplied by your employer

·   A certified copy of the death certificate

·   A certified copy of the marriage certificate

·   A service certificate from the employer

·   Proof of banking details

·   FORM UF126 – completed

If you are the deceased’s child making the claim, you must register at the nearest Labour Office.

Documents you need to make the claim:

·   Your ID book

·   Copies of the deceased’s last six pay slips

·   UI-19 – information supplied by your employer

·   A certified copy of the death certificate

·   A certified copy of your birth certificate (as the deceased’s child)

·   A service certificate from the employer

·   Proof of guardianship

·   Proof that you are a learner who was dependent on the deceased

·   FORM UF127 – completed

The Labour Office will give you Form UF128 which needs to be filled in by the deceased’s last employer and then submitted at the Labour Office.

 Only one person can claim a dependent's benefits.
